UL 產品安全申請認證所需資料
按標準要求，廠商于測試前需提供我方下列資料：
1.填寫：產品安全驗證申請客戶資料表附件
2.填寫：產品基本資料清單附件
----此清單包括所有零件之産品型號、生産廠家、零部件編號及有關參
數（電流、電壓、功率、阻燃等級等）。
----對於具有安規認證的元器件，例如：電源線、插頭、開關、溫控
器、保-險管等，請提供歐洲認證證書複印件。與安全有關之零部
件，如未具有安全認證，請提供技術參數，包括産品型號、生産廠
家、額定電流、電壓、工作溫度等，以便由我方進行隨機測試。

附件
為協助驗證單位對貴公司之產品有更進壹步的了解，請提供貴公司產
品之下列初步資料，以供正確估價或預估測試項目。若此表不便詳述貴公
司之產品，請以其他方式說明。
1.產品型號/GradeDesignation:
Ifyouwishtosubmitmorethanoneformulationpleaseprovideauniquedesignationfo
reachformulation.
2.原料名稱/GenericPolymerMaterial:
如矽膠 siliconeorsiliconerubber,PC,PP,ABS,PMI,……等,foreachformulation。
若是含填充物，請註明最大及最小百分比。若是共聚合物(如分子量不同)，也請註
明成份重量百分比。

3.防火等級/FlammabilityClass:
(譬如:94V-0,V-1,V-2,HB,VTM-0,VTM-1,VTM-2,HBF,-2,5VA/5VB)
Vtest：啟動開關後，不需使用者持續操作即可自行運作的家電產品，其所使用的
塑料需通過 V燃燒測試。
HBtest：燈具及使用時必須由使用者持續操作的電器(家電)產品，如 hairdryer等，
必須要通過 HB燃燒測試。
HBFtest：針對發泡塑膠材料使用于電器產品及其他裝置的零件上(可是不用於結構
支撐)
VTMtest：同 Vtest，只是產品厚度太薄,約 .
4.最大及最小厚度:(mm)
MaxThickness(mm)
如果只申請壹個厚度，將來只能生產該厚度+10%誤差以內的產品。

5.產品運送型式/FormofShipment:
如顆粒、薄片、粉末、液體……….等。
(Forexample,pellets,sheets,powder,twoliquidcomponents,finishedproductset
c.)
6.所欲申請顏色/Colors:
申請 allcolors請提供 naturalcolor(原料原色)，白色，黑色，及紅色,allcolors
代表以後能够生產任何顏色。
Natural,White,Black,andRedwillrepresentallcolors.
7.產品成分百分比/MaterialComposition:
建議提供。

如 silicone及 siliconerubber皆已經認定為 150℃，若要申請認證超過 150℃，則
需 要 另 測 LongTermThermalAgingTest(LTTA)。 (各 材 料 RTI請 參 閱
所示)。

9.產品製造步驟/ManufacturingProcess:
建議提供。

dtoknowhowthematerialisprocessed(suchasformmolding,injectionmolding,etc.
).Pleaseadviseifpolymerizationoccursatelevatedtemperatures(besuretospeci
fycuringtimeandtemperature)
oftwoormoreliquidcomponentswewillneedaseparateidentifierforeachcomponent
,themixingratioofthedifferentcomponents,andthecuringconditions(Temperatu
re/Time)forthematerial.
10.加拿大認證/CUL:□Yes□No
UL將主動為您辦理，並不額外收費，除非您告知不辦理。CUL認證能够使您的產品進
入加拿大。

(如 ElectricalTestsorPhysicalTests等 )。 電 氣 項 目 包 括 ：
HAI,HWI,HVAR,HVTR,CTI,D-495,Volume/SurfaceResistivity(體積電阻與表面電
阻),DielectricStrength(耐壓)。如果您的產品是要用于電器成品上，可能需要做
電氣測試，此項由客戶自選，但若沒，您的產品可能不能用於電器成品或將來需要
時，則能够另行補測，但 IdentificationTest(成份/配方比對)可能需再測壹次來
與先前測試結果比對。請與您的客戶聯絡,了解他們的成品(如電視、電腦螢幕、電
話,烤箱,烤麵包機,……………….,etc)于向 UL申請該成品安全認證時，需要何種
額外的安全測試項目。
HAI： 產 品 將 來 會 與 非 絕 緣 材 料 (liveparts)接 觸 或 接 近
arcingparts-½innon-arcingparts–1/32in
HWI：產品將來用于 endproducts上時會與非絕緣材料(liveparts)接(觸)近於
1/32in以內
HVAR：不同 endproducts上使用時，會與非絕緣導體材料接觸時均需要申請，視
endproducts產品類別而定。
CTI：同 HWI條件但
1. 導體材料有正向極/磁性。
2. 金屬材料于使用時有接地。
3. 表面暴露於使用中，能接觸得到。
D-495：產品用於高電壓電視產品上。
VR/SR及耐壓測試：產品用於電器絕緣上或非絕緣導體材料之外殼。

12.相關注意事項:
第壹次工廠生產檢驗/INITIALPRODUCTIONINSPECTION(IPI):
于工程部門完成報告後，UL會通知當地的檢驗中心至貴工廠進行第壹次生產檢驗
(IPI)，完成 IPI後，檢驗員會將臨時的報告交給您，您即授權能够使用 UL的標誌。
Onceourengineeringinvestigationhasbeencompletedandyourproductisdetermine
dtocomplywithourrequirements,
presentativewillthencontacttherepresentativeatyourmanufacturerandarrange
odeterminethatyourproductisappropriatelydescribedinourreportandthatyouar
ordertoavoidanydelayspleaseprovidethefollowinginformationforeachmanufact
urerthatwillbeproducingamarkingyourproductwiththeULRecognitionmark:
1). Completenameandaddress.
2). Name,telephone,andfaxnumberofthefactoryrepresentative.
3). DateyouintendtobeginproductionofULRecognizedFabricatedParts.
季檢驗/FOLLOW-UPSERVICES:
UL檢驗員將會每季不定期至貴工廠檢驗及每年抽驗樣品複測。
